- 2 weeks before the flight date, call 877–487–2778 to schedule your appointment. Can’t call any earlier. They will not grant you a spot.
- If you get an appointment, it will be within 72 hours (3 business days) of your travel date. I got my appointment less than 48hrs before departure.
PREPARATION FOR APPOINTMENT:

1. Fill out DS11 FORM (or the form attached in your confirmation email)
2. Passport photo (2 photos)
3. Printed proof of international travel
4. Birth Certificate (for kids application)
5. Photocopy of Birth certificate (front & back. 1 copy)
6. Parents ID: Driver License (or US Passport) for both parents
7. Fees Check or Money Order (someplace do accept credit card)
8. $80 passport book + $60 expedite + $35 execution fee (2 checks $140 + 2 checks $35. Name and DOB on check) (check on any update on fees)

1. Have ID ready before entering the building for security check
2. Security takes 3–5 mins
3. Ask for direction to the 3rd Floor Passport elevator
4. 3rd Floor Security:
4.1. have 4 digit Confirmation# ready (or use phone number that you created the appointment)
4.2. Appointment time (they will check off list)
4.3. Present ID
5. Only one person wait in line (the rest of the family need to sit down) [wait time 10–15 mins]
6. Call up to the window [5–10 mins]
7. Passport application (make sure 2 pages NOT front and back printout)
8. Get the new generation of passports with thicker ID page or thicker book. Same price.
9. Passport photo (2 photos)
10. Birth certificate (original)
11. Birth certificate (photocopy)
12. Parents Passport (original)
13. Parents Passport (photocopy)
14. Expired passport
15. Flight ✈️ itinerary (1 copy per application)
16. Get a number and wait to be called [30 mins]
17. Called up to window 🪟[15 mins to process paperwork]
18. Call the rest of the family up. For kid’s passport, both parents must be present
19. Kids show face without mask
20. Parent (dad signed) swear and affirm and sign
21. Then mom signed and double-check info
22. Pay — accept credit cards & checks
23. Passport will be ready to pick up in the afternoon after 3:30 pm. Only one parent needs to come to pickup
